You can find the 2025 health assessment survey on our website at cloverhealth.com/you or the My Clover member portal. You can complete the survey online or download a printable version. You also received the survey in paper form in your Clover Member Welcome Kit.

Plus, you can receive a reward for registering, logging in, and navigating to the LiveHealthy app (this reward can only be redeemed once).

The payment can be processed once we receive confirmation that you have completed the Getting to Know You survey, “Get Active,” and Preventive Care reward activity. Funds should be available on the Visa card within 72 hours. The One Pass® reward payment can take up to 60 days, and the LiveHealthy Visit can take up to 90 days.
