Georgia & South Carolina	All	12/22/2022	None	There is an SEP available to Medicare members, particularly Veterans, who want to disenroll from their Medicare Prescription Drug Plan (PDP) in order to maintain or enroll in another type of creditable drug coverage such as Veterans Administration (VA) or TRICARE. Members who wish to enroll in, or maintain other creditable coverage (such as TRICARE or VA coverage), should select the SEP election option 'I have or am enrolling in other drug coverage as good as Medicare prescription drug coverage (like TRICARE or VA Coverage' on the online applications or indicate the SEP code 'CDC' on paper applications. Members are permitted to use this SEP within 2 months of their MAPD or PDP plan disenrollment date to enroll in an MA-only plan such as the Clover Valor plan.
New Jersey	Bergen, Morris, Passaic, Sussex, Warren	03/13/2023	05/31/2023	A state of emergency was declared for 5 counties within New Jersey due to a winter storm. As a result, there is an SEP that is effective thru 05/31/23. The 5 impacted counties are: Bergen, Morris, Passaic, Sussex and Warren counties. Note: Clover Health does not have plans in Warren county. Tennessee	Cannon, Hardeman, Hardin, Haywood, Lewis, Macon, McNairy, Rutherford, Tipton, Wayne	04/07/2023	06/30/2023	A state of emergency was declared for 10 counties within Tennessee due to severe storms that occured from 3/31/23 to 4/1/23. As a result, there is an SEP that is effective thru 6/30/23. Note: Rutherford county is the only county impacted within our service area. Georgia	All	03/26/2023	06/30/2023	A disaster has been declared for the state of Georgia due to tornadoes and severe storms. As a result, there is an SEP that is effective though 06/30/23. This SEP opportunity is ONLY available to beneficiaries who: * Reside, or resided at the start of the SEP eligibility period in an area for which a federal, state or local government entity has declared a disaster or other emergency or they do not reside in an affected area but rely on help making healthcare decisions from one or more individuals who reside in an affected area; and * Were eligible for another election period at the time of the SEP eligibility period, and; * Did not make an election during that other valid election period due to the disaster or other emergency. As a result, there is a disaster SEP that is effective thru 09/30/23. The affected counties are noted and the counties within Clover's service area noted in bold. Andre, Bandera, Bell, Bexar, Blanco, Burnet, Cameron, Colorado, Comal, Comanche, Coryell, Eastland, Ector, Edwards, Erath, Gaines, Gillespie, Hamilton, Hays, Hidalgo, Jeff Davis, Kendall, Kerr, Kimble, Lampasas, Llano, Mason, Matagorda, Medina, Midland, Mills, Parker, Real, Travis, Waller, Wharton, and Williamson counties.
